Gout attack right now? Here is tonight's plan.

No sales pitch on this page. The pain is real, you are not being dramatic, and nobody fakes being unable to stand a bedsheet on their toe. Here is the calm version of what to actually do for the next few hours, from someone who has spent these exact nights staring at the ceiling.

The next hour, in order

Get the weight off it. Sit or lie down and put the joint up on a pillow, higher than your heart if you can manage it. Gravity is quietly pumping more fluid into that joint every minute it hangs down.

Get everything off it. A bedsheet on an inflamed toe genuinely feels like a car parked on it. Prop the blanket on a laundry basket or leave the foot outside the covers. You are not being dramatic, light touch really does hurt that much.

Ice it, never heat it. Ice pack or frozen peas wrapped in a thin towel, never straight on skin. 15 to 20 minutes on, at least 20 off, repeat as often as you want. A flare is already hot, and heat makes it angrier.

Drink a full glass of water now. Then keep a bottle in arm's reach and sip all day. You will not flush the flare out by chugging a gallon, so do not try. Just stay steadily, boringly hydrated.

No alcohol until it settles. Especially beer. Not one to take the edge off. This is the best proven trigger link there is, and this is not the week to test it.

If your doctor gave you a flare plan, follow it. Whatever rescue plan or prescription they set for exactly this situation, that plan wins over anything on this page. If you already have something from a doctor for exactly this, tonight is what they gave it to you for. Take it the way they told you, not more. If you have nothing and this is your first one, that is the reason to be seen today.

Sleep with the foot outside the covers, up on a pillow. Accept a bad night. It is one night. Stairs and walking wait until the joint says so, not until you feel brave. I pushed walking too early once and paid for it with a second week.

For texting someone mid flare, or pasting anywhere links don't work.
The single most important don't.

Do not start, stop, or change uric acid medication on your own because of a flare. Flares in the first months on a uric acid lowering drug are common and expected, and stopping one suddenly tends to trigger the exact thing you are trying to escape. If a doctor wants to start or adjust something tonight, that is their call to make with you, and the guidelines do allow starting during a flare in some situations. What is never right is you making that change alone. The one thing that is not a flare and does not wait is a new rash, mouth sores, or a fever after starting a new medication. That is a same day call to whoever prescribed it.

When this is not a wait it out night

One more thing, because I know where your head might be. At 3am the pain tells you this one is different, that it is climbing and will never stop. I have been exactly there. That feeling is real and nearly everyone with gout has had it, and it is wrong. Flares peak, then they resolve, almost always within a few days to a week or two. Yours will follow the same curve even though it does not feel like it right now.

You are not powerless while you wait. Rest, elevate, ice, water, no alcohol, follow the plan your doctor set. The fire always passes.

After tonight. The honest curve, day by day

Nobody told me about the tail of a flare, so I walked it scared. Here is the shape most of them take. Yours may run shorter or longer, and none of this is a drug schedule.

The first night. The worst of it. Hot, red, swollen, and the sheet hurts. Everything above is for tonight. The question in your head is whether this one ever stops. It does.

Days one to three. The peak. Pain often climbs a little more before it turns, which is exactly when the brain says it is different this time. Keep the foot up, keep the ice going, keep the water going. If a fever shows up at any point, that is a same day visit, not a wait and see.

Days four to ten. The turn. Redness fades first, then the heat, then the swelling, and the joint starts to feel like yours again a little at a time. Most flares resolve inside a few days to a week or two. A flare that is not budging after several days of doing everything right is a doctor visit, not a test of toughness.

The weeks after. This is the part nobody warns you about. You get a real pain free day, take a proper walk to celebrate, and the next morning the joint is stiff and sore. No fire, just not right. Most of the time that is a joint coming back online after weeks of you protecting it, and it eases with short walks before long ones and flat ground before hills. The honest other half is that years of repeated unmanaged flares can leave lasting marks. So watch direction and time. If weeks pass and it is not trending better, or its motion stays clearly limited next to the other foot, bring it to a doctor and say exactly that. The flare ended weeks ago and this joint still is not right.
