A gout attack is one of the more painful things a body can do to a person. The joint is hot, red, swollen, and the weight of a bedsheet hurts. They are not exaggerating. They may be short with you, and they may be scared in a way they will not say out loud, because at 3am the pain tells you it will never stop. It does stop, and almost everyone with gout has lain there thinking those exact thoughts.
A pillow to get the foot up, higher than the heart if they can manage. Then leave the foot outside the covers, or prop the blanket on a laundry basket so nothing touches it.
Ice in a thin towel. Frozen peas work. Fifteen to twenty minutes on, at least twenty off, as often as they want. Never heat. A flare is already hot.
A full glass of water, then a bottle within reach. Steady, boring hydration. Not a gallon at once.
No beer to take the edge off. Not one. It is the best proven trigger there is, and this is not the week to test it.

"Isn't that an old man's disease?" My wife's version was that gout is for out of shape seventy year old alcoholics. Most people with gout heard something like it, and it lands as blame. The truth is kinder. Gout is largely genetic. It comes down mostly to how efficiently the kidneys clear uric acid, and the body makes most of its uric acid inside itself regardless of diet. Fit thirty year olds get it. Women get it. Nobody did this to themselves with a plate of shrimp.
The other one is "just lose weight" or "just stop drinking". Both can matter and neither is the whole story, and both are the sentences that make someone stop talking to you about it.
Get them to get the number. Gout is a uric acid problem before it is a toe problem, and the single most useful thing after a flare is a blood test and the number itself, not just the word normal. A self ordered test costs around fifty dollars and needs no doctor visit. When they have a result, the free interpreter below reads it in plain words, and the doctor conversation gets specific instead of vague.
The thing I would have wanted from the outside was simple. Believe me, get the foot up, and do not make it a character flaw. You are already doing the first part by reading this.

Most flares peak in the first days and resolve within a few days to a week or two. The weeks after can be stiff and sore as the joint comes back. A joint that is not trending better weeks later is a doctor visit.
